Report a safety concern, a near miss, or a good catch from the field. Include the date, location, and what you saw people doing.

1. Stop work if anyone is in danger. Report after the hazard is controlled.

2. Critical step: a task step where an error would cause immediate, irreversible harm.

3. Emergency? Call 911 first, then your supervisor.
